Brooklyn Plaza Medical Center is proud to operate a School-Based Health Center (SBHC) at Benjamin Banneker Academy, providing accessible, high-quality healthcare to students right where they learn. Our SBHC offers a wide range of services, including routine check-ups, immunizations, sports physicals, chronic condition management, behavioral health support, and acute care for illnesses or injuries.

Students can receive care at the SBHC regardless of whether they already have a primary care provider outside the school. Our goal is to complement, not replace, their regular doctor by ensuring immediate, convenient access to essential healthcare services during the school day.

Enrollment Requirements

To enroll, students must meet the following requirements:

1. Be enrolled in one of the Thomas Jefferson Campus schools.
2. Complete a Parental Consent Form (click the link below to access all required documents).
3. Provide a copy of the child’s immunization records.
4. Submit the completed consent form to B06 or the school’s main office.
